A Taxi Medical, or Taxi Driver Medical, ensures private-hire and hackney carriage drivers in the UK meet the required health standards (D4) for the profession. 

When applying for your taxi or private-hire vehicle (PHV) licence, or renewing an existing one, you must provide a certificate to indicate you have passed this assessment. After your appointment, we issue you with this certificate within 48 hours, subject to individual health circumstances.

    What does a Taxi Medical involve?

    Our GMC-registered doctor will check your fitness to drive, through an initial questionnaire and a physical exam. This may include non-invasive tests, such as:

    • Health questionnaire
    • Blood pressure test
    • Physical examination
    • Eye test
    • Medical records review
    • Urine sample
    • Hearing test

    Preparing for your appointment

    We understand that you want your appointment to go as smoothly as possible. To give yourself the best chance of success, make sure you turn up with everything you need.

    Although requirements vary depending on the local authority, you must always bring the following:

    • Photo ID
    • Glasses, if you wear them (and details of prescription)
    • Hearing aids, if you wear them
    • A list of any prescribed medication
    • A driver medical form: either the DVLA D4 Group 2 Driver medical form, or that of your local council, with the relevant sections completed
    • A copy of your medical records: Full or summary, depending on local authority requirements

    On application, then every five years up to 65 years of age. Yearly thereafter.

    Dacorum Borough Council

    Records from the past five years.

    Do I need to complete any forms before the medical?

    DVLA D4 Group 2 Medical Examination Report.

    How often is the medical required?
    On application, then every three years up to the age of 65. Yearly thereafter.

    Liverpool City Council

    What medical records do we need?
    Full.

    Do I need to complete any forms before the medical?
    Liverpool City Council Medical Assessment.

    How often is the medical required?
    On application. From age 45, every five years, and yearly after the age of 65.

    Knowsley Council

    What medical records do we need?
    Full.

    Do I need to complete any forms before the medical?
    The Knowsley Council Taxi Licensing Service Hackney Carriages and Private Hire Drivers Medical Certificate, and the DVLA D4 Group 2 Medical Examination Report.

    How often is the medical required?
    On application, then at the age of 45, and again at the age of 55. Yearly, from the age of 65.

    How often is the medical required?
    On application, then upon reaching 45. After the age of 45, you will require a medical report every five years. After the age of 65, this is required yearly.

    Full.

    How often is the medical required?
    On application, then every six years up to 60 years of age, then every three years up to age 70. Yearly thereafter.

    St Helens Metropolitan Borough Council

    On application. Once you reach 45 years of age, you will need to renew every five years, then yearly from the age of 65.
